This will make the vaccination process easier for you– and for us!

Bring your paperwork

If you are able to print this consent form, please fill it out in its entirety and bring it with you to your vaccination appointment.  This will save you time and minimize the amount of time your appointment takes.

Mark your calendar

Understand that you’re making a commitment.  The Moderna vaccine requires two shots.  If you get your first shot, you must return to the same location in 28 days for your second shot.  If you’re not willing to make that commitment, please wait to schedule your appointment until you can.
