What to bring with you?

When admitted to the Swing Bed program, you will want to bring clothing that is easy to get off and on and is comfortable for exercising and activities of daily living training.  Shoes that are comfortable on your feet are also essential.

You are also encouraged to bring items like iPads, pillows and books that will help create a more comfortable, homey atmosphere.

Admittance + discharge:

Admits are done on a case-by-case basis.  Medicare and most insurances cover the cost of swing services, but your individual situation will be investigated prior to your admission. Medicare requires that you have a consecutive three day qualifying hospital stay to be eligible for coverage and admission to swing must be within 30 days of your acute hospitalization. Your case manager should be able to assist you with any coverage questions you might have.

Once admitted, our dedicated care team will work with you to develop an individualized treatment plan with your specific needs and discharge goals in mind.  The care team will monitor your improvement from day to day.  As you advance toward your goals, the care team will begin preparing for your discharge and ensure that you and your family are set up for success.

The Swing Bed coordinator will arrange for discharge needs depending on the ongoing care and support needed and work with the therapists to ensure all necessary accommodations have been made at the home to keep the patient safe.  The care team will make sure your family is comfortable with new adaptive equipment, home exercises and procedures that support your success at home.
